Document forms and layouts
Prepare clear application forms and check how issued documents will look.
Choose a document service
Barangay administrators manage the services offered through Document Forms and the related service settings. Check the document name, instructions, availability and fee. Changes should match the barangay’s approved process.
Set the information applicants must provide
Use the form editor to add or update the questions shown to applicants.
- Give each field a clear label, such as Full name or Purpose.
- Mark a field as required only when it is needed to process the request.
- Add short instructions where an applicant may need help.
- Review the form from an applicant’s point of view before publishing changes.
Prepare the document layout
Where a document layout upload is available, use the supported Word document format and the template instructions shown in the editor. Keep the barangay heading, document text, official details and signature areas clear.
Preview before using a layout
Use the preview to check names, dates, long addresses and the purpose text. Confirm that no text is cut off, the signature areas remain clear and the verification code is readable when included. Save the changes only after reviewing the result.
Check changes after saving
Open the service again to confirm that the intended form and layout are in use. If a preview fails, check the file and required fields or ask your administrator for help before issuing a document.
